Dear Patients,

It is with great difficulty that I announce that I will be closing my practice as of August 31st, 2026. I will stop seeing patients in the office mid August. There will be a formal letter sent to all of my patients very soon. Because of this, I will no longer be accepting new patients or new pregnancies for new OR current patients.

You may choose any physician you wish to transfer your records to. I am referring to Metro OBGYN and Walnut Lake OBGYN . They are aware of my practice closing and that I will be referring patients to them. However, I am unsure of what insurances which doctors take. You can find a physician by calling the number on the back of your insurance card and asking for OBGYNs in your area, contacting Huron Valley Sinai Hospital and asking for a physician referral, or visiting dmc.org and clicking on “Find a Doctor”.

Continuity of your care is of the highest importance for both me and my staff. We will do everything we can do to make the transition to whichever physician you choose smooth and stress free. We will be happy to copy your records and have them ready for you at the office to pick up before we close for good in August. In order to have those records copied and ready to pick up before we leave, you will need to fill out a record release authorizing us to release your medical records. You can do this by filling out one at your new physician’s office, stopping by our office and filling out the form, or filling out the form on our website. Records will be copied- free of charge- and available to pick up in office. As of August 15th, records will no longer be in our possession. We will be transferring them to a medical records storage facility. If you want your records from us before we close the office, we must receive your request by August 1st in order to process them. The office will remain opened until August 31st for any questions you may have or to pick up records that were requested. After that, you will be able to contact the record service to obtain your records. Please be aware that the record service will require a fee to copy or send your records. Details of that company and their pricing will be updated soon.
